چکیده

OBJECTIVE   The objective of this study is to evaluate the safety and feasibility of the AZUR hydrogel-polymer coated platinum coil (hydrocoil) for vascular occlusion in patients with congenital cardiovascular disease (CCVD). DESIGN   Retrospective case review. PATIENTS   Eight patients with CCVD who underwent attempted vascular occlusion procedure with the hydrocoil at the University of Michigan between January 1, 2008 and March 1, 2011. Median patient age was 1.6 years (range 0.6-27.5 years) and median weight was 10.6 kg (3.9-74.8 kg). OUTCOME MEASURES   Complete vascular occlusion of the target vessel was assessed at procedural completion with angiography and noninvasive imaging at follow-up. Key procedural variables and adverse events were also evaluated. RESULTS   A total of 21 hydrocoils were successfully placed in seven vessels, in six patients. Vessels included four (57%) veno-venous collaterals, two (29%) aortopulmonary collaterals, and one (14%) main pulmonary artery. Hydrocoils could not be placed successfully in two patients, including one baffle leak and one veno-venous collateral. Pretreatment of the hydrocoil with steam to soften the coil, prior to vascular introduction, increased the likelihood of successful implant (100% vs. 60%). Complete occlusion was observed in 100% of vessels either at the time of catheterization or at follow-up. There were no complications attributable to the hydrocoil. CONCLUSIONS   Hydrocoils are safe and effective devices that can be utilized for vascular occlusion in a variety of blood vessels in patients with CCVD. Coil pretreatment may increase the likelihood of procedural success.

The eJournal of the European Society of Minimally Invasive Neurological Therapy

INTRODUCTION: Bare platinum coils have demonstrated safety and efficacy in the treatment of intracranial aneurysms; however, a persistent and important limitation occurs in the form of delayed recanalisation. The authors report their multiple-hospital, single-practice experience with the use of first and second-generation hydrogel coils for the treatment of ruptured and unruptured cerebral aneu...

Acquired Infrarenal Abdominal Aortic Coarctation: Treatment with Percutaneous Self Expandable Stent

Abdominal aortic coarctation is an extremely rare vascular pathology. Its etiology can be congenital or acquired. Here we present a case of acquired infrarenal abdominal coarctation in a 66-year-old woman who complained of low back and legs pain. She had no signs of resting lower limb ischemia, with diminished distal pulses and normal blood pressure in upper and lower extremities. Magnetic reso...

THERAPY AND PREVENTION VASCULAR ANOMALIES Coil embolization of congenital thoracic vascular anomalies in infants and children

When significant thoracic vascular anomalies occur in children, they may present surgical difficulties making operative management undesirable. The recent development of a new, accurate coil-delivery system has enabled us to embolize 17 vessels in five children by passing Gianturco steel coils coated with thrombogenic Dacron strands through No. 5 or 6F end-hole catheters suitable for infants. C...

Treatment of unruptured intracranial aneurysms using internally expanding coils.

BACKGROUND AND PURPOSE The International Subarachnoid Aneurysm Trial (ISAT) showed that patients with intracranial aneurysms treated with coil embolization have better clinical outcomes than those undergoing neurosurgical clipping. However some patients treated endovascularly have recurrence of aneurysms. Low packing density is often cited as a reason for recurrence. Coiling with hydrogel cover...
